126. Selected applications were incorrectly approved. One application as approved as free and should have been reduced based on household size and income, one application was approved as free and should have been reduced based on a student being counted twice on the application. One application was incorrectly denied due to incorrect income calculation.

Please provide the steps the Sponsor will take to make this correction and ensure future compliance. For example, how will the Sponsor ensure all applications approved are accurate (i.e., have a second reviewer periodically check applications for accuracy).

Finding:
502. Signage must include the requirement to select a fruit or vegetable. Additionally, the most recent health inspection was not posted.
Corrective Action:
The finding was corrected by the SFA while the SA was on-site. The SA ensured compliance by posting the most recent health inspection and offer versus serve signage.

Finding:
410. Menu production records and other supporting documentation indicate that required quantities of food were not available. Compliant meal documentation is maintained by the sponsor nor are standardized recipes used to determine component contribution. The chili does not have any compliant documentation in order to determine the component contribution.

Please provide the steps that the SFA will take to make this correction and ensure future compliance. For example, explaining how you obtain and maintain compliant product documentation. Include the following supporting documentation with your response: compliant documentation for chili or potential menu change to be in compliance.

Finding:
603. Standardized recipes are not used for most recipes. Standardized recipes must include, at minimum, recipe title, recipe category, ingredients with amounts, directions (including equipment needed, cooking temperatures, HACCP process/procedures, cooking, cooling and holding temperatures), portion size, and recipe yield.
Corrective Action:
Please provide the steps the SFA has taken to address the issue and will ensure future compliance. Please provide examples (at least two) of updated recipes.
Technical Assistance:
603. Recommend sponsor utilize existing menu software to adopt current recipes, menus, productions records. It will streamline current operations and ultimately eliminate manual work while providing more accurate activities of production, inventory and ordering.
By adopting standardized recipes, operations for all staff (especially for newer staff) will be very clear by stating what should be served with a menued entree and what exact serving size is to be provided to ensure component quantities are met.

Local School Wellness Policy -

1006. How does the public know about the results of the most recent assessment on the implementation of the Local School Wellness Policy?
Finding:
1005. The sponsor has not completed the triennial assessment of their Local School Wellness Policy.
Corrective Action:
The finding was corrected by the SFA while the SA was on-site. The SA ensured compliance by sharing the plan for conducting the Local Wellness Policy Assessment.
It appears the last review of the implementation of the wellness policy was in 2019, which was the year before COVID. The District spent last year developing a new five year strategic plan for the district with three pillars or areas of focus. One of the pillars of our strategic plan is wellness. This year we have focused on implementing year one of the strategic plan. Going forward, we will plan to develop an assessment of our wellness policy and build it into our next year’s action steps for the wellness plan.

Finding:
1105. Foods and beverages sold on the school campus during the school day to students (that are not a part of an exempt fundraiser) do not meet the Smart Snack standards. The oatmeal and chocolate cookie sandwiches, pop tarts, BodyArmor Lyte, and Capri Sun are not compliant.
